Purchase of 230 opioid trauma kits from High Threat Innovations, LLC of Rancho Cucamonga, California as a sole source vendor. The total amount of $50,912 includes custom opioid trauma kits designed specifically for first responders to support remediation activities in the City of Orange.

6.                     Discussion and Background

In 2022, California Attorney General Rob Bonta announced settlement agreements with opioid manufacturer Janssen Pharmaceuticals and Johnson & Johnson, along with distributors McKesson, Cardinal Health, and AmerisourceBergen. In 2023, he proposed multistate settlements with manufacturers Teva and Allergan, and pharmacies Walgreens, Walmart, and CVS. These settlements resolve over 3,000 opioid-related lawsuits from state and local governments nationwide, providing substantial financial resources to combat the opioid crisis in California.

California cities and counties are expected to receive up to $1.8 billion for substance use prevention, harm reduction, treatment, and recovery from the settlements with Teva, Allergan, and the Pharmacies, in addition to $2.2 billion anticipated from the Janssen and Distributors settlements. Payment timeframes may vary and can extend up to 18 years. Funds must be specifically allocated to opioid remediation activities focused on prevention, intervention, harm reduction, treatment, and recovery services within California communities. To date, the City of Orange has received $1.2 million from these settlements.

To prevent overdose deaths and reduce harm, the Police Department will equip officers with specialized full-size and micro-opioid trauma kits designed to provide rapid response to opioid overdoses and trauma injuries. Opioid overdoses can lead to several types of injuries and medical complications, primarily due to respiratory issues. Common injuries include trauma from falls or accidents resulting in head injuries, fractures, and facial injuries. These kits include life-saving components such as naloxone (Narcan) for opioid overdose reversal, tourniquets tailored for high-risk operational environments, and other wound care supplies.

Currently, patrol fleet vehicles are equipped with two Narcan treatments per vehicle which are replenished upon use. Officers in non-patrol assignments such as detectives and special units do not maintain an inventory of Narcan. Full-size trauma kits will be distributed to all public safety officers while custom micro trauma kits will be distributed to the specialized response team. Remaining inventory will be maintained in the Training Bureau of the Police Department for new incoming officers. Training and first aid refreshers on the use of the items will be facilitated within the Police Department.

These kits include life-saving components including the following:

                     Medical-grade naloxone for opioid overdose reversal

                     Tourniquets

                     Vented chest seals

                     Gloves

                     Hypothermia blanket

                     Pressure dressing

                     Tape

                     Shears

                     Quick clot gauze

                     Custom packaging suited for tactical carry and deployment

High Threat Innovations, LLC is the sole known source that provides opioid trauma kits specifically engineered to meet the operational needs of law enforcement, combining both overdose intervention and trauma management in a compact, ruggedized form. Other vendors do not offer the required integration of opioid and trauma response capabilities in both full-size and micro formats. 

Additionally, this procurement is of critical urgency due to a recent increase in opioid-related field incidents and trauma threats. Delays in procurement could lead to preventable fatalities or serious injuries. High Threat Innovations, LLC has confirmed immediate fulfillment capability, including expedited shipping and readiness to provide field training support upon delivery. No alternative vendors can guarantee the same speed and level of readiness for both kit formats.

Staff has researched available competitive options and, with approval of the City Manager, certifies the customized first responder opioid trauma kits are offered by only one vendor. High Threat Innovations, LLC in Rancho Cucamonga, California, is the sole manufacturer of the TraumaPAK and is the sole service provider offering a limited lifetime guarantee on their products.

The Police Department recommends the purchase of opioid trauma kits by High Threat Innovations, LLC in the amount of $50,912. An appropriation of $50,912 from the Opioid Settlement unreserved fund balance is necessary to cover the cost of the trauma kits.
